To attend DAILY AND SHABBAT services at the Spanish, Chevra Shaas and Maghen Abraham, proof of double vaccination is MANDATORY until February 21, 2022. For Shabbat services proof of double vaccination must be submitted by Friday 10 a.m. before Shabbat. This is a one-time only procedure; once we have this form, it will be kept on file.

Unvaccinated individuals will NOT be admitted.
